Health Insurance for 1099 Workers in North Carolina: The Complete Guide
If you're paid on a 1099, nobody is handing you benefits. That's the bad news. The good news is you have more control over your health insurance than most W-2 employees — if you know the two markets.
Step one: figure out your subsidy status
HealthCare.gov subsidies are based on modified adjusted gross income — your 1099 income minus business expenses, half your self-employment tax, and your retirement contributions. A contractor grossing $90,000 can easily have a MAGI of $65,000 and qualify. Estimate honestly; you reconcile at tax time.
Step two: if you don't qualify, look at private plans
Above the subsidy line, full-price marketplace plans in North Carolina are expensive and often HMO. Private shared-network PPOs on Cigna, Aetna and PHCS PPO are priced on your age and health, enroll any month, and cost healthy contractors substantially less. The trade-off is underwriting: a short health questionnaire, and serious conditions can be declined.
Step three: don't forget the deduction
North Carolina has a flat state income tax, so the self-employed premium deduction helps at both the federal and state level. Self-employed people deduct health premiums above the line, which reduces both income tax and the amount used to calculate your subsidy.
Timing for 1099 workers in North Carolina
Marketplace: November 1 – January 15, or within 60 days of a life event (losing a job's coverage counts). Private: apply by the 20th for the 1st of next month, any month. North Carolina has no state individual mandate.
Common 1099 situations in North Carolina
Realtors in Charlotte and Raleigh, contractors and tradespeople, truck drivers and delivery drivers, nurses on contract, consultants, and gig workers all land here. Medicaid expansion in late 2023 moved hundreds of thousands of lower-income North Carolinians onto Medicaid — which means the people still shopping are mostly above the subsidy range.
