Can I Get Health Insurance Without Obamacare in North Carolina?
Yes — and it's a legal, regulated market, not a loophole. Here's what's actually available in North Carolina outside the ACA marketplace.
Private shared-network health plans
These are medically underwritten plans sold by carriers that lease access to national PPO networks (Cigna, Aetna and PHCS PPO in North Carolina). They cover doctor visits, hospitalization, surgery, prescriptions and preventive care, with deductibles and out-of-pocket maximums like any major medical plan. Because they underwrite, they can price healthy people lower than the marketplace can.
What to be careful about
Not everything sold "outside Obamacare" is real insurance. Fixed-indemnity plans pay a set dollar amount per day and leave you exposed to large bills. Health-sharing ministries aren't insurance and aren't required to pay claims. Discount cards are not coverage. Ask one question of anything you're offered: what is the individual out-of-pocket maximum? If there isn't one, it isn't major medical.
North Carolina-specific rules
Medicaid expansion in late 2023 moved hundreds of thousands of lower-income North Carolinians onto Medicaid — which means the people still shopping are mostly above the subsidy range. North Carolina has no state individual mandate.
Who should stay on the marketplace
Anyone who qualifies for a subsidy, and anyone with a significant pre-existing condition. Private plans can decline you; HealthCare.gov can't.
